Community Paramedic

Empress is seeking a Community Paramedic to join its Community Paramedicine/Mobile Integrated Healthcare team. Community paramedics provide high-quality, compassionate in-home and community-based care to patients who may not require emergency transport but need acute or non-acute treatment in place, as well as ongoing or follow-up medical care and support.

Working in collaboration with primary care providers, emergency departments, physician practices, hospital systems, social services, and healthcare teams, community paramedics help reduce unnecessary emergency room visits and improve patient outcomes. They also support chronic disease management, preventive care, and post-hospitalization follow-up.

Community paramedics can also perform all functions of other Empress EMS NYS EMT-Paramedics.

Key Responsibilities

  • Conduct advanced in-home patient assessments, including physical exams, medication review, safety, and health education.
  • Provide urgent/unscheduled patient medical responses.
  • Provide treatment-in-place through protocols and physician collaboration.
  • Provide support for chronic disease management (e.g., diabetes, heart failure, COPD).
  • Follow up with recently discharged hospital patients to ensure recovery and adherence to discharge plans.
  • Provide ongoing clinical monitoring and interventions.
  • Assist patients with access to social and community services.
  • Collaborate with healthcare providers to support and implement patient care plans.
  • Maintain accurate and timely documentation of all assessments, interventions, and referrals.
  • Monitor patients for changes in condition and escalate care when needed.
  • Participate in public health initiatives, health screenings, and community outreach programs.
  • Educate and coach patients and families on health maintenance, disease prevention, and self-care strategies.
  • Respond to emergency 911 calls when appropriate to support the EMS system.

Qualifications:

Education: High school diploma or GED required

Licensure/Certification:

  • Certified Paramedic (NREMT-P or NYS EMT-P Certification)
  • Completion of a community paramedicine program, or community paramedic training will be provided for qualified applicants
  • BLS and ACLS, PALS (Pediatric Advanced Life Support) certification required

Experience:

  • Minimum of two years of experience as a field paramedic
  • Experience in community health, primary care, or public health is an asset
